Having trouble using the on-line consultation questionnaire?
1. Make sure you're using Internet Explorer (MAC or PC) browser and not Netscape (AOL), Mozilla, Safari etc. This form doesn't work with broswers other than Internet Explorer.
2. Internet Explorer isn't set-up to process emailing forms. You need to go into your Internet Explorer settings (mac: preferences) and enter the appropriate setting in the "email" section. Note, these are for the Internet Explorer browser and shouldn't be confused with setting up your regular email client application.
3. If you like, you can download a printable form and fax or mail it to us.
